Special Note:  New Jersey law mandates that all children who are 59 months and younger and enrolled in preschool be vaccinated with the flu vaccine annually by December 31.
 
 
PHYSICAL EXAMINATIONS
 
Upon entry to school, a physical is required and must be documented on our physical form.  The exam must be within the previous 365 days.  Scroll to the bottom of this page for a link to the district physical form.
 
The Howell Board of Education recognizes the importance of medical examinations at least one time during each of student's developmental stages.  These stages are Preschool through Grade 3, Grade 4 through 6, and Grades 7 to 12.
 
If you do not have insurance physicals and immunizations are available at no cost.  For additional information please call the Monmouth County Immunization Program at 732-431-7456 x8580.  Clinics take place at the Monmouth County Health Department located at 50 East Main Street in Freehold.
